ATTENTION PENNCREST PARENTS

Registration for children entering kindergarten for the 2009-10 school year in PENNCREST School District will be held on Friday, April 24, 2009, in each elementary school. Children should be present at registration. Kindergarten activities are scheduled for them to participate in.

Children must be five (5) years old on or before August 31, 2009 to register. During registration, parents will complete health and registration forms. Birth certificate and immunization records must be presented at the time of registration.

Pennsylvania State Law requires every child entering school for the first time to have the following number of doses of each of these immunizations:

4 DT (Diphtheria, Tetanus) with the fourth dose after age 4

3 Polio

2 Measles after age 1

2 Mumps after age 2

1 Rubella after age 1

3 doses of Hepatitis B

2 doses of Varicella (chickenpox) immunization or history of disease.

If your child’s immunizations are incomplete, please call your physician or the Crawford County State Health Center
